The Form below will outline some details that are necessary to confirm as you are having Laurie minister at your church or event. If a particular section is not applicable to your event please indicate by placing “NA” (Not Applicable) in the space on the form. Upon confirming the information you have submitted by email, I will forward the email back to you for your records. Your date is only confirmed after this form is completed and submitted to the ministry office. If we do not receive a completed form we cannot confirm the date and details of your event.
Please be advised that on rare occasions, depending on the event and our arrangement with you, we may ask for a deposit to secure our traveling costs and hotel stay during a tour.
Prior to submitting the booking checklist please be certain that you have already confirmed the details with myself or another ministry representative.
